Why has my blueberry bush died

I have had it just over a year and it has flowered and had fruit on. The other bush I have is doing really well.

If you have treated this bush in the same way as the other one, all I can suggest is that something is perhaps eating its roots...have yu checked the rootball....it may be vine weavil.
