My sister lives in Shropshire and has in her garden this unusual foxglove can you tell her anything about it please

It is a normal native foxglove and the unusual flower is due to a mistake in the flower formation. A bit like a Siamese twin. The plant will flower normally next year.

I had a delphinium that kept doing it and even seen it in a Buddleja. It is unpredictable and mostly an unattractive feature.
