my potatoes grew to be about 4 feet tall, however when I
dug some up today, the potatoes are very small and have
got lots of tiny holes with tiny worms in. What is the
cause?

Small potatoes could indicate a lack of water or that you have dug them up too soon. If they are main crop they need to be in the ground for around 16 - 18 weeks. Early potatoes 12 - 14 weeks. The tiny worms are wire worm or eel worm I would think.
