after feeding the lawn with autumn lawn food we have noticed that we now have patches of mould on the grass and when removed the grass underneath is brown and dying. is this a disease

You may have put too much lawn feed down in places and it's gone mouldy, lightly scarify your lawn, do not cut it on a low setting, after it has rained it should recover.
