You may need to spot treat the patches with weedkiller then re-sow with good grass. This may take time, remember to use a weedkiller that will go down into the roots, but not stay active in the soil. So try one with glyphosate in. Keep the lawn closely cut otherwise to discourage it. And don't compost the trimmings if you use weedkiller.
