These are usually bought as standards, but if you want to create one,you need to select a central stem which is straight, then cut off all the growth till you get to the top, where you leave growth in place and prune to shape. If the stem you choose already has branches coming off it,it will always have a knobbly appearance though, and you will always have to clip off new growth.
