Do the leaves of the apple tree have brown spots too and are the apples that drop appear rotted on the inside? If not rotted on the inside, it could be a fungus; if rotted it could be a bacterial attack. In any case this tree has big problems that will require extensive treatment. You are from the USA so I would contact your state agricultural extension service for further advise on exactly what the problem is and how to treat it.
